SURVEY INSTRUMENTS
	Household Survey Version 1
	Questions 1-14 on Version 1 and Version 2 are the same. These questions cover neighborhood isssues, quality of life issues, and transportation issues. Version 1 has unique questions about housing/basic needs, family/individual problems, health and health care.
	Household Survey Version 2
	Questions 1-14 on Version 2 are the same as Version 1. These questions cover neighborhood, quality of life, and transportation issues. Version 2 has unique questions about employment/job skills, dental care, family conflict, caregiving, older adults, parenting, youth problems and child care.
